We want to welcome you as a new patient to our dental practice and also tell you a little about ourselves. Hillcrest Dental Group began as a solo practice in 1965 and has expanded over the years to become one of the most active, traditional practices in Sonoma County.

Drs. Richard Thayer, David Straub, Leah DeBerardinis, and Steve Nagle now head a staff of highly trained professionals. The goal is to provide our patients with the highest level of ethical, caring service. We do not advertise but depend on word of mouth referrals from satisfied patients for our success.

We provide comprehensive care for the whole family and have built our reputation on honesty and quality in all that we do. We treat our patients as though they are part of our family.

To the right of this page you will find a two-sided acquaintance and Medical History Form to give us important information about you and to acquaint us with any medical precautions we need to take on your behalf. It would be very helpful if you would fill out both sides of this form prior to arriving for your appointment. This will give you time to adequately complete the form and will give the doctor more time to spend with you. Please remember to bring the completed form with you on the day you are scheduled.

If you have insurance, please remember to bring your insurance card with you. As a courtesy to our patients we submit claims after each visit. We are contracted with Delta Dental and United Concordia, but we accept any insurance that will allow you to see the provider of your choice.

INITIAL VISIT ~ Your initial visit will include a comprehensive analysis of your dental health, a review of your medical history and an evaluation of any special dental needs you may have. We will only take those x-rays needed to complete a diagnosis and will discuss our findings with you at this first visit.

FINANCIAL CONSIDERATIONS ~ All patients have their own special set of circumstances regarding dental treatment and its cost. We will provide you with an estimate for the treatment required and discuss treatment and payment options with you. We pre-authorize with your insurance company for extensive dental work and help you with your forms, and may extend credit when needed. We accept Visa, Master Card, Discover Card, and American Express. We also offer CareCredit services.

SCHEDULED APPOINTMENTS ~ We work with scheduled appointments and reserve that time just for you. If it is necessary to change your appointment, please give us 24 hours notice.

HOW TO REACH US ~ Our office is open Monday through Friday from 8:00 AM to 5:00 PM. There is always a doctor on call. If you need help with a specific question, ask for Marie, our office manager or Lisa, our office supervisor.
